Homeowners who purchase a minimum of 12 Simonton windows or patio doors that are eligible for the Federal Energy Tax Credit under the American Recovery and Reinvestment Act of 2009 from any of the qualifying product lines from a participating Simonton Windows^® dealer between February 1, 2010 and December 31, 2010 will qualify for this offer to receive a certificate good for up to $150 toward preparation of a 2010 personal income tax return at a participating H&R Block office in the United States (the “Certificate”.) Redemption forms must be postmarked within 60 calendar days of invoice date. Any claims postmarked after 60 calendar days of the invoice date will not be processed. In order to qualify for the Certificate, Homeowner must (1) complete all information on the redemption form and attach copy of invoice(s) with qualifying products and dates of purchase circled or highlighted and (2) mail the redemption form and copy of invoice(s) to the address on the form in a timely manner as outlined above. If these Terms of Offer are not met, the redemption form will not be processed and no Certificate will be issued. Redemption forms may be copied as needed for multiple purchases. Forms are also available from participating Simonton dealers and online at www.simonton.com/hrblock.

Certificate valid only at participating H&R Block offices in the United States for an original 2010 personal income tax return. Certificate is valid January 1, 2011 through October 17, 2011. Void if sold, purchased or transferred, and where prohibited or restricted by law. The Certificate cannot be transferred or used by anyone other than the qualifying homeowner. If the Certificate is improperly transferred or if the Certificate is improperly used for the preparation of an income tax return for someone other than an eligible person designated by Simonton Windows, the user agrees to reimburse H&R Block for the value of the Certificate. Distributors and their employees are not eligible. Other terms may apply to the use of the Certificate.
